By yourself

1. Complete the form for release of patient information(download here )
2. A copy of ID card/ driving license/ Passport
   In the case of a minor, please bring a copy of birth certificate or house registration book which shows the minor’s name.

By representative/ others

1. Complete a form for release of patient information (download here) and give a power of attorney to an authorised person indicating his/her full name น
2. A signed copy of ID card/ driving license/ Passport which is still valid
3. A copy of ID card/ driving license/ Passport which is still valid of the authorised person
