1. Perfect Diamond Health Plus card does not offer discount coupon on IPD room rates.
2. Prime Platinum card does not offer discount coupon on OPD doctor fee.
3. OPD doctor fees discount coupon does not valid at Bangkok Hospital Sanamchan, Bangkok Hospital Phitsanulok, Phyathai Hospital 1, 2, 3, Nawamin and Paolo Hospital Group.
4. This promotion cannot be used in conjunction with any other promotions or privileges.
5. Chivawattana reserves the right to change any privileges without prior notice.
6. Member must present their Chivawattana card in order to redeem a discount.
7. In case of privilege transfer, the recipients must bring Chivawattana card or a proxy letter from card owner in order to receive full benefits.
8. Per visit discounts cannot be combined for the same treatment, unless stated otherwise.
9. If for any reason any service specified is not at one hospital, the privilege can be used at any other participating hospital within the Bangkok Hospital Group, Samitivej Hospital Group, BNH Hospital, Phyathai Hospital Group
and Paolo Hospital Group, unless the service is only available exclusively at a specific hospital.
10. Members cannot use Chivawattana benefits for medical services covered (discounted or otherwise) by their employer’s health policy with participating Bangkok Hospitals.
11. In case of lost or damaged card, please inform your hospital for a new card. There will be a fee of 100 Baht.
12. If the member decides to cancel a card, there will be no refund.
13. The hospital reserves the right not to allow the use of any Chivawattana privileges with other promotional programs or employer benefits program; unless otherwise stated, Chivawattana discounts apply only to regular priced exams.
14. Chivawattana card reserve the right to alter any privileges without prior notice.
